I have found that the best fitting bra's are from Marks and Spencer. They have just started shipping to the US and Canada. That might be a way to go (dismantal one of their bra's). They are known for great support. If you need any help PM me.

I'd go with Lane Bryant if you want a store where you can walk in and buy ready made in "hard"cup larger cup sizes. Problem is that I've never seen sew-in cups suitable for a ballroom costume any larger than D. I'm going to go with DSV and whoever said earlier in the thread, buy a ready-made bra with the cup size you need and get the dress designer to dismantle it and position the cups.

sometimes the obvious is so obscure. tis a neat solution
in some dresses I guess one could sew much of the bra itself in too...
